You know, now that I think of it, back to front might work because we're working with whole pages, not like we're writing from right to left or bottom to top. I might try it once.

You could equally go with alternating pages. List on left, notes on right. These don't even need to be synced to progress at the same page. If your notes run ahead too far, you can easily adapt by using both sides. Or, go with Gerry's system: Notes and lists on every page, but list-items are marked to stand out.

Hi,
I like this way to handle my todos and calendar.
Maybe we can give some tips and tricks how we work with this system so we improve the way we work with it.
I use a pocket moleskine.
Draw a calendar for next 3 month - over 2 pages - . If one week get "full" and create a week calendar - 1 page -
I use AF4 method for my todos.
Here is my first 2 cents

One challenge I'm having is keeping track of all my "links" to my next pages. I started out trying to update them every time I dismissed a previous page but that was a lot of scribbling out and there was the potential to miss pages if I didn't do it correctly (my poor page 38 got orphaned for a couple of days as a result of such an error). So now if I have C/D pages in between open ones I'm not bothering and just flipping quickly past those inactive pages. I indicate their C/D status at the top of the page so it's not hard to tell.
(And now i don't have to dismiss page 56 because I did one item on it.)

This happens to me too if I have a numbered link. I have other pages between my Autofocus pages. I often will check all the pages from back to front.
I am past page 190 in my Moleskine. It has 240 pages. So I am approaching the end in a week or two. There is a thread here on First Notebook Filled that has some tips on how to handle this situation.

Yeah, I've done several of Mark's other systems analog-style so the whole new notebook thing won't be a huge deal for me when I get to that point. Some stuff will have to migrate to Evernote and I'll have to carry two books for a little while (and OfficeMax had the notebooks that I like for more than half off today! I bought three.) but the transition hasn't ever been bad for me before.

I currently have a DWM2 list in the front and my notes at the back. I had tried out AF4R earlier and didn't love having my pages mixed. But as you said, markhedm, I was unwilling to divide my notebook into sections since it's impossible to tell how long each section would be and I don't like wasted pages.
I'm also not sure mixing my list pages with my notes would work as well as if I was using AF1 - DWM2 is more like a really really long list independent of page length.

I start writing my notes from the back-to-front, so I would stop when the list and notes met in the middle. The disadvantage is it feels a little funny writing notes backwards.
